Birth Certificate: First copy $20.00, each additional $3.00. Check or money order must accompany a signed request with the following information: Child's birth name, date of birth, place of birth, Mother's maiden name, and Father's name.Marriage Certificate: First copy $20.00, each additional $3.00. Check or money order must accompany a signed request with the following information: Bride's maiden name, groom's name, date of marriage, and place of marriage.
Death Certificate: First copy $20.00, each additional $3.00. Check or money order must accompany a signed request with the following information: Name of deceased, date of death and place of death.
