Prenatal Care Coordination

Goals:

To reduce incidence of complications of pregnancy and preterm / low birth weight babies.

Services Provided:

We provide case management to Medicaid eligible pregnant women who are considered at risk for adverse pregnancy outcomes.

• Education to high-risk pregnant women on: self-care, fetal growth and development, smoking cessation, nutrition, preparation for labor and delivery, and breastfeeding.
• Referral for financial counseling, housing, nutrition, school completion.
• Postpartum education and referral on self care and recovery, infant care and feeding, family planning, well child care, immunizations, parenting.
